Diagnostic APFT Failure Counseling Statement Example
Failing the Diagnostic Army Physical Fitness Test (APFT) is a clear indication of a Soldier’s substandard level of fitness and a red flag for the leader to step up the current PT program and get the Soldier on track for the record APFT. It is imperative that your Soldiers are aware of the consequences of failing the record APFT, such as losing the privilege to use tuition assistance, attend WLC, receive awards and all favorable actions due to being flagged, this information should be documented on DA Form 4856 Developmental Counseling Form along with DA 705 APFT Score Card immediately after failing the diagnostic APFT.
