Lost Military ID Card Counseling Statement Example
Use this Counseling Statement Example when a Soldier loses his or her military id card or Common Access Card CAC.
The Soldier will have to report the lost id card th the provost marshal and get Counseled on DA 4856 by the First Sergeant or Company Commander prior to receiving a new military id card. Some installations have special rules for reporting and replacing a lost military id card so be sure to check with your local MP station for more information.
